Disability Empowerment Board, Central Government. invite to submit an online application for a unique identity card for people with disabilities. The central government will issue UDID cards and disability certificates to Divyang (Viklang) residents. Candidates can even check and track the status of their UDID card application and download the electronic Disability/e-UDID card online. To use the UDID card, all Persons with Disabilities (PWDs) will need to apply online at swavlambancard.gov.in.

Unique Identity for People with Disabilities is a flagship project of Prime Minister Narendra Modi's government. The government is implementing a project to create a national database and issue a unique identity card for people with disabilities. People with Disabilities (PWDs) can apply online for their Disability Certificate/UDID card. Before completing the online application, check who can apply and review the procedure instructions.
UDID Card / Handicap Certificate Online Application Form
This project will ensure transparency, efficiency and easy transfer of government benefits in Divyangjan. This UDID card will also help track the actual and financial progress of the beneficiaries at the village, block, district, state and national level. Here is the entire online registration process:
STEP 1: Visit the official website swavlambancard.gov.in
STEP 2: On the homepage, click at “Apply for Disability Certificate & UDID Card” link or directly click http://www.swavlambancard.gov.in/pwd/application
STEP 3: The Unique Disability ID Registration Form / Disability Certificate Form will appear as follows:-
STEP 4: Here candidates have to fill all the necessary details accurately and upload “Photo” and “Signature” and click at the ‘Next’ Button. Then candidates have to enter the ‘Disability Details’, ‘Employment Details’, ‘Identity Details’
STEP 5: Finally, candidates can click at the “Submit” button to complete the Person with Disability Registration process to apply for UDID Card / Disability Certificate.

After application process, candidates can Update their Personal Profile and download e-UDID Card & Disability Certificate by making UDID Card Login

Track Application Status of UDID Card Online
Applicants can fill the UDID Card Application Form Online and can also apply through e-mitra or Atal Seva Kendra to get national disability identity card. Here candidates have to enter their “Enrollment / UDID / Registration No.” and click at GO Button to track their application status. After the application is approved, candidates can download their e-UDID Card & Disability Certificate by making Login.

UDID Card Benefits
The UDID card shall bring a host of benefits to the Persons with Disabilities as given below:
- Persons with disabilities will not need to make multiple copies of documents, maintain, and carry multiple documents as the card will capture all the necessary details which can be decoded with the help of a reader
- The UDID card will be the single document of identification, verification of the disabled for availing various benefits in future
- The UDID Card will also help in stream-lining the tracking of the physical and financial progress of beneficiary at all levels of hierarchy of implementation – from village level, block level, District level , State level and National level
Candidates can see the objectives of UDID Card Project. After getting UDID Card, all the disabled candidates will receive all the govt. schemes benefits in a transparent manner.

Till Date, 10 states have started generating UDID Cards, there are 4 states / UTs where UDID Project is expected to start soon. As of now, there are 22 more states / UTs where UDID Card project is yet to start.

Important Points to Apply Online for Disability Certificate / UDID Cards
- Persons with Disabilities (PwDs) have already started applying online for Disability Certificate/UDID Cards from all States/UTs.
- Enrolment /Application Registration number will be generated for all the applications submitted successfully.
- SMS confirmation will be sent to the applicant with the enrolment/application registration number
- Applicants who do not have Disability Certificate will receive SMS with date and time to visit the nearest CMO office for assessment
- The Disability Certificate /UDID Card will be generated online after assessment and verification
- SMS confirmation will be sent to the applicant with the allocation of UDID number
- The status of the application can be checked online by enrolment/application registration number
- Printed UDID Card will be sent to the PwDs on their Postal Address as and when printed (Process of printing the card is yet to start; will be intimated on the website).
